ACL Reconstruction Recovery: Phases Explained and What to Expect

ACL reconstruction is a lengthy but highly successful procedure. Understanding the rehabilitation phases — and what drives progress at each stage — makes all the difference in achieving a full, confident return to activity.

Overview: How Long Does ACL Recovery Take?

Full recovery after ACL reconstruction typically takes 9–12 months. Return to recreational sport is usually possible from 6–9 months; return to competitive or high-level sport generally requires 9–12 months or more. The timeline is driven not by arbitrary time points, but by biological graft maturation and measurable functional criteria.

Phase 1: Early Post-Op (Weeks 1–2)

  • Pain and swelling management using ice, elevation and medication
  • Regaining full knee extension (straightening) — a critical early priority
  • Gentle range-of-motion exercises — target 90° flexion by week 2
  • Quadriceps activation — quad sets and straight leg raises
  • Walking with crutches, progressing off crutches as comfort allows

Phase 2: Strengthening (Weeks 2–6)

  • Progressive quadriceps and hamstring strengthening
  • Closed-chain exercises — mini squats, step-ups, leg press
  • Stationary cycling (once flexion permits)
  • Balance and proprioception training
  • Full weight-bearing walking without crutches

Phase 3: Neuromuscular Control (Weeks 6–12)

  • Single-leg exercises — step-downs, single-leg squats
  • Advanced balance training
  • Lateral movement introduction
  • Swimming, cycling and light cardiovascular training

Phase 4: Return to Running (Months 3–6)

Running is typically introduced at 3–4 months, beginning with a structured return-to-running programme (walk-jog intervals, progressing to continuous running). Criteria-based clearance — based on limb symmetry indices for strength and hop tests — determines readiness, not time alone.

Phase 5: Return to Sport (Months 6–12)

Sport-specific drills, agility training and reactive movement are introduced progressively. Return to competitive sport requires passing functional performance tests demonstrating >90% limb symmetry alongside psychological readiness.
